Job Description:
InGenesis is currently seeking a dedicated Community Health Worker II (CHW II) to work with our client. In this role, you will support and assist patients across various locations and community events. As a trusted liaison between patients and healthcare providers, the CHW II will help bridge gaps in care by connecting individuals to essential services and resources.

Key Responsibilities
•Conduct outreach, engagement, and care coordination activities.
•Enroll patients in health plans using CEC certification.
•Analyze screening tool results and communicate findings with the Care Team.
•Link patients to internal and external services (e.g., food, housing, transportation).
•Participate in team meetings, case conferences, and training sessions.
•Maintain accurate and confidential documentation in compliance with agency and funder requirements.
•Support grant deliverables through assessments and data collection.
•Provide immediate crisis intervention and advocacy as needed.
•Use de-escalation techniques and program evaluation tools.
•Educate patients on health topics such as nutrition, hygiene, and substance abuse.
•Work in diverse settings including clinics, schools, mobile units, and patient homes.
•Attend workshops, trainings, and meetings as required.
•Perform additional duties as assigned by the supervisor.
Qualifications
•High school diploma or equivalent required.
•1-2 years of experience in community-based healthcare preferred.
•Bilingual skills preferred.
•CHW Certification required.
•CEC Certification required.
•Proficiency in computer and internet-based applications.
